Avoiding "Never
Never" Land: A Patient Safety Conference that gets Back to Basics
Sponsored by CAMC and WVMI
The goals of this symposium are to highlight "never
events" in medical care that, according to the National Quality Forum, are
clearly identifiable, preventable, and serious in their consequences for
patients, and that indicate a real problem in the safety and credibility of a
health care facility. "Never
events," like surgery on the wrong body part or mismatched blood
transfusion, cause serious injury or death to beneficiaries, and result in
increased costs to the facility to treat the consequences of the error. As part of ongoing efforts to provide better care, we will be
focusing on ways to reduce or eliminate the occurrence of "never
events" - serious and costly errors in the provision of health care
services that should never happen. This program is planned for all physicians, nurses, hospital
